Assisted living providers are realizing both the benefits and challenges of meeting the rising acuity needs of their residents. Advancing age and chronic diseases are challenging providers to consider how to best meet those needs. A chronic disease management framework offers opportunities to expand clinical capabilities and integrate with existing service models. Trends in programs, clinical capabilities and services as well as resources and tools will be explored.
